How to proceed in dealing with damages

  1. In the event of a damage event, contact the AXA Assistance service and follow its instructions. The assistance service is available nonstop.   2. Report the damage event to the insurance company, preferably online, and provide all necessary documents.

  3. We will investigate the damage event based on the submitted documentation and inform you of the result.

  • identification data of the insured person
  • if the incident was investigated by the police, fire brigade or other authority, give its address or reference number
  • attach any supporting documents proving the occurrence of the event e.g. invoices for medical care provided

Detailed description:

  • Required documents

    • original receipt of payment for medical treatment, if an invoice is issued, the original of this invoice
    • bills issued by a doctor or by a pharmacy on the basis of a prescription from the treating doctor (original receipt), indicating the amount and subject of reimbursement
    • receipts for medicines and medical devices (crutches, spectacles, orthoses, etc.) must always be accompanied by a prescription (copy possible) issued in the name of the insured person, stating the date of issue, quantity and description
    • medical report, results of examinations, etc.
    • in the case of the insurance type “Repeated stay”, a written document proving the insured’s stay in the Czech Republic for a period of less than 90 days before the occurrence of the insured event abroad. This proof may be provided by an air ticket or a ticket issued in the name of the insured, indicating that the journey was made from the Czech Republic, a travel document certificate, etc.
  • Medical expenses insurance

    • original bills issued by the doctor stating the amount and subject of payment, including the original receipt for payment of the bill
    • the original unpaid invoice
    • original bills for medicines and medical devices, including a copy of the prescription
    • medical report indicating diagnosis, results of examinations, etc.
    • in case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. a flight ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
    • police report if the incident was investigated by the police
  • Accident insurance
    Permanent consequences:

    • medical report from abroad
    • medical reports from the Czech Republic from the course of treatment, including medical reports from rehabilitation
    • in the case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. an air ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
    • police report if the incident was investigated by the police

    Death by accident:

    • death certificate
    • medical certificate of the cause of death
    • in the case of the agreed type of stay “Repeated” – proof that the insured’s continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. an air ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
    • police report if the event was investigated by the police
  • Insurance of personal belongings

    • police protocol
    • photographic documentation of the damaged or destroyed items or the site of the incident (e.g. rooms or vehicles that have been ransacked)
    • list of stolen (destroyed) items
    • original purchase documents of the stolen (destroyed) goods
    • in the case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. an air ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
  • Cancellation fee insurance

    • a copy of the service order or travel contract
    • confirmation of the reason why the paid service had to be cancelled (e.g. medical report)
    • proof of payment for the service (tour, air ticket, hotel, etc.)
    • a statement of cancellation fees from the service provider
    • proof of the amount refunded to you by the service provider
    • the cancellation policy of the service provider
  • Liability insurance – damage to property

    • police report if the incident was investigated by the police
    • photographic documentation of the damaged or destroyed goods or the site of the incident
    • written statements from victims and witnesses
    • original purchase documents of the destroyed (damaged) goods
    • evidence of the repair of damaged goods
    • in the case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. an air ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
  • Liability insurance – personal injury

    • police report if the incident was investigated by the police
    • complete medical records of the injured person
    • written witness statements
    • a written claim for compensation by the injured party or a claim by the injured party
    • a written statement by the insured as to fault and the amount of compensation
    • decision of a state authority (prosecutor, court, expert opinion)
    • in the case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. an air ticket from the Czech Republic, a certificate of a visit to a doctor in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
  • Mountain service intervention insurance

    • original confirmation of payment of the mountain service intervention, or original unpaid invoice
    • the mountain service intervention report
    • in the case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the event did not exceed 90 days, e.g. a flight ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
  • Insurance in case of hospitalisation

    • medical discharge report from hospitalisation abroad
    • in the case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. an air ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
  • Travel document insurance

    • original documents proving the amount of the damage (the fee for issuing a replacement travel document abroad, transport and accommodation related to the issue of the replacement travel document)
    • in the case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. a flight ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
  • Baggage delay insurance

    • original documents of handover of luggage to the air carrier
    • written confirmation from the carrier of the delayed delivery of the baggage, including the date, time and duration of the delay
    • original documents confirming the purchase of items necessary for the basic necessities of life
    • in the case of the agreed type of stay “Repeated” – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. an air ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
  • Vehicle delay insurance

    • written confirmation from the carrier of the delay in departure, including the date, time and duration of the delay
    • the ticket/ticket
    • original documents proving the cost of meals, luggage storage and accommodation

    • documents proving the cause of the incident (e.g. police report, statement or certificate from the carrier)

    • in the case of the “Repeat Stay” type of insurance – proof that your continuous stay abroad before the occurrence of the event did not exceed 90 days, e.g. an air ticket from the Czech Republic, a certificate of a doctor’s visit in the Czech Republic, a certificate from school or employment in the Czech Republic, any official document from the Czech Republic, etc.
